Payment is due at time of service unless you are approved for a Payment Plan. Any insurance payments will go directly to you.

INSURANCE

Our office understands the value of insurance benefits to our patients. We will provide a completed dental claim form and stamped envelope as a courtesy to you along with all documentation to assure your maximum benefits. There are no guarantees, however, because the insurance relationship is between you and the insurance company. To insure maximum benefits be sure to read your dental plan benefit booklet provided by the insurance company prior to your appointment and discuss all concerns with your insurance company. Payment for dental services is the responsibility of the patient.

USUAL AND CUSTOMARY RATES

Our practice is committed to providing the best treatment for our patients. We charge what is usual and customary for our area and for the high quality materials we use. Your are responsible for payment regardless of any insurance company's arbitrary determination of usual and customary rates.

MISSED APPOINTMENTS

Appointments are valuable blocks of time. When an appointment is broken or cancelled on short notice (less than 48 hours) it prevents us from helping someone else. Wasted appointment time also results in higher health care fees. In order to control costs for all our patients, we must charge a non-refundable $70.00 cancellation fee for all appointments that are cancelled with less than 48-business-hours notification (Monday - Friday). As a courtesy to our patients, we make every effort to contact scheduled individuals to remind them of their appointment. Please help us control costs as well as serve you better by keeping all scheduled appointments.
